To ensure quality and accurate care, it is important that our patients understand our care policies. All of our patients current and new are required to review and complete acknowlegement of these policies as well as the Health Insurance Portability and Accountability Act (HIPAA) required by the Department of Health and Human Services.

Billing Policy
Payment is expected at the time of service. Co-payments and deductibles must be payed at the time of service. Contracted insurance payments are accepted if the information is presented at the time of service.

We participate with most local insurance plans. If we are not listed as a provider on your plan please call prior to your visit and we can help to confirm coverage.

We encourage all patients to check their own benefits to get specific information concerning their policy. For example, patients should know whether their policy covers preventative care visits, problem visits, or both.

As a courtesy we will bill your insurance company on your behalf. Insurance information must be on file with proof of insurance provided by the patient. We reserve the right to ask for credit card information as a back-up in case insurance does not cover the services. We also reserve the right to charge a 1.5% service charge on all accounts greater than 90 days or after all insurance payment attempts have been exhausted.

Appointment Policy
We see patients by appointment only. Please call our office to schedule an appointment as soon as you identify the need so as to have the best chance of getting the time you desire. Same day appointments and referral appointments are frequently available however time slots are not always available. Same day appointments are distributed based on physician schedule and severity of the problem.
